Welcome to Ethos Training Systems, LLC (“Ethos,” “we,” “us,” or “our”). These Terms of Service ("Terms") govern your access to and use of our services, including personal training, group fitness classes, virtual online training, and other fitness-related offerings. By using our services, you agree to these Terms in their entirety. Please read them carefully.
1. General Membership Terms
Ethos provides memberships for various fitness services, including personal training, and group classes. The following terms apply to your membership:
No Proration for Unused Time: There is no proration for unused time during any given month. If you miss classes or training sessions, you will not be credited for the unused portion of your membership.
Cancellation at Any Time: You can cancel your membership at any time with no cancellation fee. To cancel, simply send an email to info@ethostrainingchi.com. Cancellations are effective immediately, and no further charges will be made after cancellation.
Cancellation and Suspension Requests: To cancel or suspend your membership, please submit a request at least fifteen (15) days prior to your next scheduled auto-draft date by emailing info@ethostrainingchi.com. Requests made after this period may not take effect until the following billing cycle.
Membership Suspension: The minimum suspension period for your membership is 1 month, and the maximum suspension period is 3 months. During this period, your membership will be temporarily paused, and you will not be charged for the suspended time.
2. Reservation and Cancellation Policy for Classes and Training
When you reserve a spot for a class or training session, we hold that spot specifically for you. We ask that you be respectful of our policies for cancellations and no-shows:
Cancellation or Rescheduling of Reservations: If you need to cancel or reschedule your reservation, please be sure to cancel your spot at least 2 hours prior to the scheduled start time. Cancellations made after this 2-hour window, or no-shows, will result in a $25 fee.
No Show Fee: If you fail to attend your scheduled session without notifying us in advance, a $25 fee will be charged to your account.
3. Payment and Billing
The following terms apply to payments and billing for your membership:
Declined Credit Cards: If your credit card is declined for any reason, a $10 processing fee will be charged. It is your responsibility to ensure that your payment information is up-to-date.
Monthly Rate Changes: Your monthly membership rate is subject to change. If Ethos decides to adjust the membership fees, we will notify you in advance, prior to your next billing cycle. You will have the option to review and accept the new rates before being charged.
4. Waiver and Release of Liability
By using Ethos’s services, including personal training, group fitness classes, virtual online training, and access to our facilities, you agree to assume all risks associated with physical exercise and wellness activities. You will be required to sign a Waiver and Release of Liability that outlines the risks involved and releases Ethos from liability in the event of injury or damage sustained during your participation in our services.
5. Photo and Video Release
You grant Ethos the right to take photos, videos, or other recordings of you during training sessions or classes, which may be used for marketing or promotional purposes. You waive any right to inspect or approve these materials and release Ethos from any liability related to the use of your image or likeness.
6. Health and Safety
It is your responsibility to ensure that you are physically fit and able to engage in the activities provided by Ethos. If you have any health concerns or conditions, please consult with a healthcare professional before participating in any fitness or wellness services. You acknowledge that the use of our facilities, including any equipment, sauna, or cold plunge, is at your own risk.
Sauna and Cold Plunge: If you plan to use the sauna or cold plunge facilities, you must follow all safety guidelines provided by Ethos. Ethos is not responsible for any injuries or health issues arising from the use of these facilities.
7. Modification of Terms
Ethos reserves the right to modify or update these Terms at any time. Any changes will be communicated to you via email or through notices posted in our facilities. Continued use of our services after any such modifications will constitute your acceptance of the updated Terms.

9. Governing Law
These Terms are governed by and construed in accordance with the laws of the State of Illinois, without regard to its conflict of law principles. Any disputes arising under or in connection with these Terms shall be subject to the exclusive jurisdiction of the courts located in [Insert County], Illinois.
